Got one of these on EBay for £2.70, delivered.
You write your details on the supplied card, before waterproof sealing then, and sticking them to your helmet.
Simple, but effective.

Also have one fitted to my lid, but didn't pay anything as work had a few left over. Will need to use a new one when I replace my lid next year.
Always good to have something that provides your details no matter what it is.

I have ICE details as a notification on the smartphone. This way if the screen is visible, then the details are easily seen. No unlocking of the phone necessary.
I also have a ICE card stored on the jacket which was given to me by Klim when I bought their Badlands jacket. It sits in a special pocket on the sleeve, so should be good in case of accident.

Thanks to the OP... didn't know these existed.
I'm an electrical engineer, and bought some of these for all my guys hardhats. I also put some on my wife's helmet and my helmet for motorcycling.
The vendor makes them not only for hardhats, but kits for motorcycles which includes a wallet card.
The only thing I did different on the motorcycle helmets was place the stickers on the sides of the helmets, not the back. I'm assuming it's easier for EMS to see it if it's on the side and not have to flip me over.

You're welcome.
For maximum visibility, put the sticker on the front...it will probably look terrible though. On the side is a good compromise.

I do the same - on all my helmets...
